This form is for Iyengar Yoga (UK) teachers to report incidents which they feel are of concern. Please complete and submit this form as soon as possible after the incident has occurred. In an emergency, please contact the police by phoning 999, or alternatively 112 (which operates across the U.K. and Europe). The non-emergency number for the police in the U.K. is 101. By completing and submitting this form you give your consent to disclose your personal information and details of the incident that appear on this form to representatives of Iyengar Yoga (UK) who may contact you to discuss this incident further.
